Outreach

MPCA’s outreach initiatives aim to provide health centers with the latest information and to connect them with key community partners to ensure that people are aware of resources available to them and have the option to enroll in programs that support their health care needs. 

MPCA seeks out opportunities to eliminate barriers to health care and to form partnerships with state programs, organizations and funders that will expand the availability of services for the people of Maine.

Children’s Health Insurance Plan Reauthorization Act (CHIPRA) Outreach and Enrollment Grant

Six of MPCA’s member health centers are participating in this program with a goal to enroll and retain children into MaineCare. The health centers participate in quarterly calls and share their experiences in finding new and innovative ways to reach out to families and encourage them to see if their child is eligible. 

Community Partnerships

In working to remove barriers to care, MPCA looks to facilitate linkages to other community agencies that provide services and expertise that would benefit health center patients.  Area Agencies on Aging, Community Action Programs, and Healthy Maine Partnerships are just some of the organizations that we work to support in working more closely with their local health center.  If you would like to discuss how your organization can work more closely with a health center in your area, contact  Caroline Zimmerman, czimmerman@mepca.org. Enabling Services

MPCA supports health centers in providing a robust array of enabling services for their patients. One effort includes work with the Maine Health Access Foundation in their Getting Down to Basics grant. Through work with MeHAF, MPCA will address transportation, health education and patient navigation models across the state, with the intent to share best practices in each with all of our member health centers.

Health Reform Messaging

The Maine Health Access Foundation (MeHAF) is working with ten non-profits across the state, including MPCA, to support accurate, direct and effective information about  has been involved in a project to help inform health centers and their patients about how health care reform will affect people in Maine.  For more information on health care reform, visit our health reform page.

Primary Care Patient Navigation

MPCA is establishing a Patient Navigator pilot program for health centers.  This project was initiated by the MBCHP through Maine CDC with the primary purposes of:

  • Improving referrals and follow up to free health screenings for health center patients
  • Increasing screening rates for breast and cervical cancer among eligible women

Technical Assistance & Training

MPCA also provides TA/Training around topics including but not limited to: Medicare; MaineCare; health reform; dental programs; race, language and ethnicity data collection; patient centered medical home and more.
